Corrupt, unethical, and dishonest practices in papers published in scientific journals?

In the July issue of VCOT, the Editor in Chief Ken Johnson touches on a topic which one has been hearing more and more of over the past few years. That of how one can no longer assume that all peer reviewed papers published in scientific journals are credible due to the increasing incidents of corrupt, unethical, and dishonest practices such as plagiarism, falsification of data, and image manipulation that get reported via sources such as Retraction Watch.
